Referral Information

We accept referrals from all physicians across Ontario. Dr. Verma has a focused practice designation and will not contribute to outside use for FHO physicians. Please fax a standard referral to:

For CBT-i referrals, please include any sleep aids the patient is using or has previously used.

For menopause referrals, please send most recent blood pressure, mammogram, pap test, and bloodwork.

For metabolic consults, please send most recent blood pressure, and bloodwork.

As consults are all OHIP covered, wait times can be long.

For patient's struggling with sleep, please refer them to mysleepwell.ca for excellent sleep resources, including free online CBTi programs.
